Quick Eligibility Screening Required
Please select “Video Visit” to schedule a brief screening before your treatment. Book your screening anytime, preferably ahead of your desired treatment date and time. We’ll review your health history and discuss your requested service. If we identify no contraindications and your provider approves treatment, we’ll schedule your appointment together during the visit, based on availability.
Submitting a request does not guarantee eligibility or an appointment. Your provider will determine whether treatment is appropriate based on your health history and assessment.
